1. Develop a comprehensive plan for delivering optimal patient care based on case discussion.
2. Develop strategies to improve data gathering before the initial patient discussion.
3. Implement strategies for enhanced continuity and streamlining of care.
4. Improve referrals to liver transplant programs.
5. Improve communications with referring providers.
6. Improve patient outcomes through enhanced clinical decision-making and peer feedback and discussion analysis.
